Your first visit to Fort Worth Dermatology Associates involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You will find all the practical information you need, such as a map and directions to our office, practice hours, our first visit procedures and more. You can also save time by completing and printing out, or completing online, the new patient forms below prior to your first appointment. If you complete the forms online, we will have access to the forms, but if you print them out and complete them please bring the forms with you to your first visit so we may include them in your medical record.
We ask that you bring your insurance cards, your drivers license, and a list of all medications that you are currently on to your first appointment so we may also include these in your medical record.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Prepare questions for the physician prior to coming for your appointment so that we may satisfy all your concerns prior to your surgery day.
Your first appointment will be a consultation with the physician about your condition and to design a plan of treatment specific to your needs. At this appointment the physician and staff will explain the treatment plan and you will be able to ask any questions and discuss any concerns that you may have .
